TwelveStone Health Partners Joins with PREMEDEX to Build Better Path to Post-Acute Care
Company is first to offer care coordination to support its post-acute medical supply and packaged medication product suite
Post-acute care provider TwelveStone Health Partners has joined with Atlanta-based PREMEDEX and its care management solution to better assist TwelveStone’s hospital and other acute care clients.

For PREMEDEX, the alliance with TwelveStone gives its care coordination platform the ability to offer packaged medications to patients in other locations beyond Tennessee.
Initially, TwelveStone will utilize the PREMEDEX 30-day readmission reduction program, to help hospitals seeking to prevent readmissions penalties as well as focusing on improving HCAHPS scores based on a more personalized care experience.
